I've always cut off the ends and chop them up and put in a zip lock freezer bag, get as much air out as ya can....

They need to be blanched first. Prepare your beans - i.e. top and tail and slice however you want (best to not put through a slicer though, imo); most people prefer angle cut but however you want, and while you are doing that bring a pot of salted water to the boil.
When it is boiling put small batches of the beans into the boiling water, then remove after about 2mins and put into cold water with ice in it to cool them down.
This inhibits enzyme action.
Drain, bag, seal, label and freeze.

No - do not need to be blanched first - just straight into the freezer.I cut them in a bean slicer, into bags, remove air, seal and freeze.Defrost beautifully (I stir fry them).
