I use:
cocoa boiled in water
then add cream
and a dash of chilli
and a little sugar.

edited to add:
if you have it - you can use Lindt Chilli chocolate - much better result than cocoa powder :)

2 cups whole milk (not low fat if you want it creamy), 30g finest dark chocolate (at least 70% cocoa), seeds from 1/2 a vanilla pod, 1/4t chilli powder, 1/4t cinnamon, pinch of salt.In a small saucepan mix together the milk, chocolate, vanilla seeds scraped out from the pod, chilli, cinnamon and salt and heat over a low heat, stirring until the chocolate has dissolved. If you have a molinillo, froth by rubbing the handle between your hands as you would if you were lighting a fire with a stick and rubbing it on a piece of wood.Or just use a whisk to froth it up.Serve hot with a sprinkle of chilli and cinnamon on top.

ETA: I just got her book yesterday and it has the above recipe in it.
